CareBridge Companionship LLC is a private pay home care provider operating in Georgia. Where applicable under federal and state law, we are committed to maintaining the privacy of your protected health information and providing you with notice of our legal duties and privacy practices.

Our Duties

CareBridge Companionship LLC is required by law to maintain the privacy of your protected health information, provide you with notice of our legal duties and privacy practices, and notify you if a breach of your unsecured protected health information occurs.

We are required to abide by the terms of this notice. We reserve the right to change our privacy practices and the terms of this notice. If we make a material change, we will post the revised notice on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with CareBridge Companionship LLC or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
