Start from home
A free home care assessment by video
Meet our care team face to face without waiting for a home visit. We learn about your loved one, answer your questions, and send a written care plan and quote.
Licensed Georgia private home care provider. Private pay.
How it works
Four steps from first call to care plan
1
Request your assessment
Call (404) 917-3101 or send the consultation form. Tell us a little about your loved one and pick a time that works.
2
Join the video call
Join from a phone, tablet or computer. Adult children who live out of town can join the same call.
3
Talk through needs and routines
We ask about daily routines, health conditions the family wants us to know about, mobility, memory, safety at home, and goals for care.
4
Get a written plan and quote
We send a care plan with the services, schedule and price. Nothing starts until you approve it.
What we cover
What we talk about on the call
Daily routine and the times of day that are hardest
Help needed with bathing, dressing, meals and moving around
Memory, mood and behavior changes the family has noticed
Home layout and safety, shown on camera if you like
Hospital stays, appointments and the people already involved in care
What matters most to your loved one
Good to know
The video assessment helps us plan non-medical home care. It is not a medical exam and does not replace your doctor.
Available to families in Forsyth, Fulton, Cobb, DeKalb, Gwinnett, Cherokee, Henry, Fayette and Paulding counties.
Questions families ask
Video assessment: common questions
Is the video assessment free?+
Yes. The assessment is part of our free consultation. There is no charge and no obligation.
Do we need special equipment?+
No. A phone, tablet or computer with a camera and an internet connection is enough. We send you the link before the call.
Can family members in other states join?+
Yes. Several family members can join the same call, which helps everyone agree on a plan.
Can we still meet in person?+
Yes. If you prefer, we can schedule an in-home consultation instead of, or after, the video call.
How do families pay for care?+
CareBridge is a private pay home care agency. Families pay directly, and we also work with long-term care insurance and veterans benefits where they apply. We do not accept Medicare or Medicaid, including the NOW and COMP waivers.
